Notice to Wisconsin Applicants Only

Wisconsin State law Section 29.66.054(11) (b) requires that any store selling beer must have a person with an operator's license on duty during selling hours. Since this law applies to Stop-N-Go, you will be required, in most cases, to obtain an operator's license. This license is issued in your name and is usable with other employers. Therefore, you will be required to obtain and pay for your own license. Stop-N-Go will pay for license renewals. Applicants who are unable to obtain an operater's license may not be considered for hire and employees who are unable to obtain an operater's license may be terminated.

Notice to ALL Applicants

By signing below, I hereby authorize Stop-N-Go to contact, obtain and verify the accuracy of information contained in this application from all previous employers, educational institutions and references. I also hereby release from liability Stop-N-Go and its representatives for seeking, gathering and using such information to make employment decisions, and all other persons or organizations for providing such information.

If I am employed, I acknowledge that there is no specified length of employment and that this application does not constitute an agreement or contract for employment. Accordingly, either I or the Stop-N-Go can terminate the relationship at will, with or without cause, at any time, so long as there is no violation of applicable federal ot state law. No representative of the Company, other than the President, has the authority to enter into any agreement for a specified period of time or make any agreement contraryto the foregoing. Any such agreement must be in writing and signed by the President. No one hase the authority to make any verbal statements to an employee that alters the employee's employment-at-will status.

I understand that I may be required to submit to a background investigation or take a pre-employment drug test to qualify for employment. If I am offered emplyment or start work before the background investigation or drug test is completed, I understand that my employment is contingent on satisfactory results of the invenstigation and drug test.

I also understand that if I am employed, I will be required to provide satisfactory proof of identity and legal work authorization within three days of being hired. Failure to submit such proof within the required time shall result in immediate termination of employment.

I further certify that the information provided on this application is true and complete to the best of my knowledge and understand that any omission, falsification, misstatement or misrepresentation may be reason for withdrawal of an offer of employment or subsequent dismissal, if employed. I understand that any information provided may be investigated as allowed by law. I consent to the release of information about my ability(ies), employment history and fitness for employment by employers, schools, law enforcement agencies and other individuals and organizations. This consent shall continue to be effective during my employment if I am hired. I certify that, to the best of my knowledge and belief, all of the statements contained herein and on any attachments are true, correct, complete and made in good faith.
